Why we verify the product line
UnitedHealthcare is the carrier where “do you take my insurance” is hardest to answer honestly with a yes or no, because UHC is not one network. It is a family of them.
A commercial Choice Plus plan through an employer, a Navigate HMO, an Options PPO, a Medicare Advantage plan and a Community Plan Medicaid product can all arrive on a card with the same logo and behave completely differently. Some require a designated primary care provider. Some require referrals. Some have narrow networks assembled for a specific employer.
Any practice that tells you it takes UnitedHealthcare without asking which one is guessing on your behalf. We would rather spend two minutes on the phone.
Call the office with the plan name and network printed on your card and we will check it against the products we are contracted for. If we are not in network for yours, you will hear that plainly rather than after a visit.
Plans that need a PCP on file
If you have a UHC product that requires a designated primary care provider, that designation is made through UnitedHealthcare, not through us. You can do it in the member portal or by calling the number on the back of your card.
We can give you the exact provider name and address to search for so you land on the right record.
Getting this wrong is the most common reason an otherwise covered visit gets denied, and it is entirely preventable.
Costs
Preventive care, including your annual physical and routine screening, is covered in full under most UnitedHealthcare plans. Problem-focused visits apply to your copay or deductible.

For your specific deductible, copay and remaining out of pocket, call member services. We can verify network status and eligibility, but only UHC can quote your benefits.
Medicare Advantage members
Ask about the Medicare Annual Wellness Visit, which is a distinct service from an annual physical and is generally covered with no cost sharing. Many people have the benefit and never use it.

Why do you want to check my plan instead of just saying you take UnitedHealthcare?
Because UnitedHealthcare is several networks under one name. Choice, Choice Plus, Options PPO, Navigate, Medicare Advantage and Community Plan products do not all share a network. Confirming the product line is the difference between a covered visit and a surprise bill.
Does UnitedHealthcare require a referral for primary care?
Most UHC PPO and Choice Plus members can book primary care directly. Navigate and some HMO products require a designated primary care provider and referrals for specialists, which you set up through UHC.
What about AARP Medicare plans?
AARP branded Medicare Advantage and supplement plans are administered by UnitedHealthcare. Bring the card and we will check it the same way as any other UHC product.
How do I find my plan name?
It is usually printed on the front of your card under or beside the UnitedHealthcare logo, along with a network name. If you cannot find it, call us with the member ID and we can usually identify it from the eligibility response.
