Insurance Accepted in Southlake, TX
We are in network with Aetna, Blue Cross Blue Shield of Texas, Cigna, UnitedHealthcare, Humana, Curative and Medicare, along with several regional employer networks, plus Medicaid through Aetna Better Health of Texas. Plan networks change every year, so confirm your specific plan with us before your first visit. Your insurer can confirm your exact plan on the member services number printed on your card.
Carriers we are in network with
Medicaid. We hold one Texas Medicaid managed care contract, with Aetna Better Health of Texas. Texas Medicaid runs through more than a dozen managed care organizations and each is a separate contract, so if your Medicaid plan is administered by someone else we are not in network for it. Call and we will tell you plainly.
TRICARE. We see TRICARE patients and file claims as a non-network participating provider. We are not contracted with TriWest, which administers TRICARE West including Texas. Call us to confirm your coverage before booking.

No match in our list. Plan names vary a lot, so call the office at the number above with the exact name on your card and we will go through it with you.
Reading your card
The carrier is the big logo. The plan name is usually smaller, under or beside it, and that is the part that decides whether we are in network.
Being in network with a carrier does not mean being in network with every one of its products. A carrier's PPO, HMO and Marketplace plans are separate contracts, and that gap is where surprise bills come from.
If what is on your card does not match anything in the checker, call us. Plan names vary by employer and by year, and we can verify yours in a minute.

Medicare
We accept Original Medicare, plus Medicare Advantage through Aetna, Blue Cross Blue Shield of Texas, Cigna and Humana. Bring your Medicare card and, if you have one, your Advantage plan card as well.
If you are on Medicare, our Medicare Annual Wellness Visit is covered at no cost to you once every 12 months.
Self-pay
No insurance, or would rather not use it? Our self-pay prices are published, so you know the number before you book. If you are uninsured you also have the right to a written Good Faith Estimate, which we will put together for you on request.
Before your first visit
- Call us with the plan name on your card and we will go through it with you
- Bring your insurance card and a photo ID
- Know your copay and whether your deductible has been met
- On an HMO, name us as your primary care provider through your insurer first
Insurance questions
How do I know if you are in network with my plan?
Call the office with your plan name and the member ID from your card. Plan names are close enough to each other that guessing from the carrier logo alone gets people into trouble.
Do I need a referral to see you?
Most PPO plans do not require one. Some HMO plans require you to name us as your primary care provider first, which you do through your insurer rather than through us.
What will my visit cost?
It depends on your plan, your deductible and the type of visit. Preventive visits are usually covered in full. Sick visits and chronic care visits apply to your copay or deductible. We tell you what we know up front, though only your insurer can confirm your benefits.
What if you do not take my insurance?
Ask about self-pay pricing. We would rather quote you a clear number than have you skip care entirely.
Why was I billed for part of my annual physical?
If we spend part of a preventive visit managing an active problem, like adjusting a blood pressure medication, that portion is billed separately. It is a rule set by insurers, not by us, and we say so in the room before it happens.
See what to bring to your first visit, browse our primary care services, or read about our primary care Southlake practice.
