This policy covers this website only. Your medical records are governed by our Notice of Privacy Practices, which is a separate document under HIPAA and Texas law.
Who we are
Magnolia Primary Care is a DBA of Noor Wellness Clinics PLLC, at 2111 E Kirkwood Blvd, Ste 110C, Southlake, TX 76092.
What this site collects from you
Nothing you type. There is no contact form, no account, no newsletter signup and no chat widget anywhere on this site. That is deliberate. Health information typed into an ordinary web form is not protected unless the form provider has signed a business associate agreement, so we do not offer one. To reach us, call the office or use the patient portal.
Standard server records. Our host records the usual technical details of a web request: IP address, browser and device type, the page requested, and the time. These are used to serve the site and diagnose problems, and we do not use them to identify individual visitors.
Analytics cookies only. The only cookies this site sets are the ones Google
Analytics uses to count visits, with names like _ga. They do not identify you by
name and they are not used for advertising. The pages listed under Analytics below set none.
Analytics
We use Google Analytics to understand how people find and use this site: which sections get visited, how visitors arrive, and whether they go on to call or book. It is set up to keep health information out of it.
- It records the section, never the page. A visit to a page about diabetes is recorded as a visit to Conditions. The specific condition, service or article you read is not sent.
- It never receives your name, your date of birth, a reason for a visit, or anything you type.
- Google signals and ad personalization are switched off, so this data is not used to show you ads.
- It does not run at all on our booking page, this privacy policy, our terms, or our Notice of Privacy Practices.
You can block it with any content blocker or with Google's analytics opt-out browser add-on, and the site works the same either way.
Third parties that see something when you visit
Four, and we would rather name them than write "we may share data with partners."
- Google Analytics. Measures visits as described above. Google receives your IP address and the section of the site you are in, and sets its own analytics cookies. It does not receive the specific page or condition you read.
- Google Fonts. Our typefaces load from Google's servers, so Google receives your IP address when a page loads. This is how most of the web loads fonts. If you would rather it did not, a content blocker will stop it and the site still works.
- Google Maps. Our contact page embeds a map. Loading that page sends your IP address and browser details to Google, and Google may set its own cookies. No other page on this site embeds a map.
- Our hosting provider, which serves the site and keeps the server records described above.
When online scheduling is available, booking runs on a third-party scheduling system. Anything you enter there is governed by that vendor's terms and by our agreement with them, not by this page. The same is true of the patient portal.
Links to other sites
We link to insurers, health plans and other organizations for your convenience. Once you follow a link you are on their site under their privacy policy, and we have no control over what they collect.
Please do not email us health information
Ordinary email is not secure. Do not send symptoms, test results, medication details or anything else about your health by email. Call the office or use the patient portal, both of which are appropriate for health information.
Age
This is an adult practice and we see patients age 16 and older. This website is not directed at children and we do not knowingly collect information from them.
Your choices
Because we collect nothing directly through this site, there is generally nothing here for us to correct or delete on request. If you want to know what we hold about you as a patient, that is a medical records question and it is handled under our Notice of Privacy Practices. Call the office and we will walk you through it.
You can block Google Analytics, Google Fonts and the embedded map with any content blocker, and your browser settings control cookies, including the analytics cookies described above.
Changes
If this policy changes, the date at the top changes with it. Material changes will be noted on this page rather than made quietly.
