Our mission
Know the patient, not just the chart
We exist to give adults in Southlake and the surrounding towns a primary care practice where continuity is the point. A small physician-led team that knows your history, appointments long enough to get to the real question, and follow-up that happens on a date rather than a hope.
We take insurance because primary care should not be a luxury product. There is no membership fee, no retainer and nothing to join. If your plan covers a visit here, that is the whole arrangement.
Our vision
Catch it while it is still a number
Most of what shortens lives in this county is visible years ahead of time as a number on a lab report. High blood pressure, prediabetes, rising cholesterol and untreated sleep apnea are all silent, all measurable, and all far easier to treat early than late.
Our aim is a practice where those numbers get found, explained in plain language, and acted on before they become the thing that puts someone in a hospital bed.
How we practice
Four commitments that decide most of what happens here. They are the reason visits run longer and the schedule holds fewer patients than it could.
- Continuity. A small team working from one chart, so nobody re-reads your history from scratch. Book the same provider each time if you prefer.
- Time. Forty-five minutes for a first visit, and follow-ups long enough to finish the conversation.
- Plain language. Results come with an explanation, not a list of flagged values you have to interpret alone.
- Independence. Referrals go where they should, not where a hospital system directs them.

Family Nurse Practitioner
Rachael Bowen, NP
Rachael Bowen is a Family Nurse Practitioner at Magnolia Primary Care. She came to primary care the long way around, through seven years in intensive care and then post-acute medicine, and it shows in how she works. She reads trends rather than snapshots, she is hard to rush, and she is unusually good at spotting the small change that matters six months before it becomes a problem.
Rachael spent about seven years as an ICU nurse before moving into post-acute care and then into primary care. Critical care taught her to read a direction rather than a single number, and post-acute care taught her to untangle a patient arriving on fourteen medications from four specialists who had never spoken to each other. Both of those are, in different disguises, the daily work of primary care.
She also practices at Magnolia Functional Wellness, managing hormone therapy and medically supervised weight loss. Separate office, separate services, but it means metabolic and hormonal complaints get a proper workup here rather than being waved off.
She lives in Keller with her husband and their two children, plays golf with mixed results, and is a committed country music fan.

Dr. Farhan Abdullah is the Medical Director of Magnolia Primary Care and sees patients here. He is board certified in Internal Medicine and sets the clinical standards the practice follows.
Who you will see
You will see Rachael Bowen, NP and Dr. Farhan Abdullah, DO. Both work from the same chart, so whoever you see already has your history, your medications and your last results in front of them.
If you want to keep seeing the same person, book with them each time. If you need to be seen quickly, book whoever has the earliest opening. Either way the visit is long enough to use what we already know about you.
